Student Information Update

 

Indicate the information you would like to update on your record (check all that apply). Your legal name will appear on official documents, such as transcripts, account statements, tax forms, and financial aid documents. Your chosen name will appear in most other places, such as rosters, emails, myClackamas, Moodle, and Outlook. Note: Visit your User Profile within Self-Service in myClackamas to update your personal pronoun, gender identity, personal email address, and/or phone number.
