EMT Program Fall 2025: Application Form

Before starting this application please note the following information:

  • You will need to submit the "Vaccine Attestation Form" in this application. Your application will not be considered for review without the Vaccine Attestation Form. Do not upload any medical records; the vaccine attestation form is all that is needed for the application. 
     
  • You will be required to submit unofficial transcripts in this application. Please have electronic copies of your unofficial transcripts available when you fill out this application.

Step 1: Review the program webpage and advising guide prior to applying. 

  • Website information includes: Advising Guide, FAQ, schedules, cost, supplies, and program expectations (e.g., essential functions, professional behaviors, etc.) If you are interested in learning more about Financial Aid and Scholarships, please visit the CCC Financial Aid Website  

Step 2: Meeting program academic prerequisites and choosing a path 

You must select a plan of study (EMT 1-Year Certificate or the EMT Career Pathway Certificate (CPC)) during the application. 

In order to be eligible for the program, students must meet the following academic requirements: 

  • EMT-105 Intro to Emergency Medical Services
  • WRD-098 Intro to College Read & Write – or – Placement into WR-121Z
  • MTH-050 Technical Math – or Placement into MTH-060 


Note: If you have taken pre-requisite courses at another institution, you will be asked to upload your unofficial transcripts in the program application. If you need assistance with course placement assessment (e.g., MTH or WR) or have general questions regarding the program, please schedule an appointment with the admissions team at welcome@clackamas.edu 

 
Step 3: Submit program application 
  • You will need to submit the "Vaccine Attestation Form" in this application. Your application will not be considered for review without the Vaccine Attestation Form.

Step 4: Collect your vaccine records 
  • If accepted, you will bring your vaccine records to the mandatory orientation. Students who do not have proof of immunization at the time of orientation, will be deferred to the following cohort. 

  • If you need assistance in locating your vaccine records, and you received vaccines in Oregon, please connect with the Oregon State Immunization Registry (ALERT IIS) system. They can provide you copies of any records they have on file.
