The easiest and fastest way to add or drop/withdraw from your courses is through your myClackamas account within Student Planning. If you have already done this, you do not need to submit this form.
If you are requesting to register for a course that requires advisor/instructor consent due to pre-requisites, special permissions, and/or the course has already started, you are required to attach a copy of the advisor/instructor consent here. This can be an email from the advisor/instructor or a Registration Request signed by the advisor/instructor. If you do not attach required advisor/instructor consent, we will not register you for the requested courses.
